I, the undersigned applicant, hereby authorize Wyoming Machinery Company (or “the Company”) to now, or any time while I am employed, to conduct a verification of my education, previous employment/work history, contact personal references, and obtain any criminal history record information pertaining to me which may be in any Federal, State, or Local Criminal Justice agency, Motor Vehicle Report, and/or other information as deemed necessary to fulfill job requirements. This may include, but is not limited to: contacting my current and prior employers, schools and military service; review of my driver history record and criminal record, if any; and verification of my certificates and licenses, subject to the provisions of the Fair Credit Reporting Act [see note (1) below]. I understand that employment with Wyoming Machinery Company will be subject to successfully passing a criminal history record and background check.

In addition, I understand that employment with Wyoming Machinery Company will be subject to successful completion of a pre-employment drug screen. I understand that, at all times, the actual or attempted manufacture, use, sale, purchase, possession, distribution or transfer of illegal drugs, controlled substances (other than those used for bona fide medical purposes consistent with a doctor’s prescription) or use of alcohol while working, while on the premises during the operating hours of those premises (including meal breaks and other rest periods), or while using or operating WMC equipment or vehicles, is strictly prohibited and that failure to comply will result in termination. I understand that I may be subject to random drug and alcohol testing during the course of employment, and a confirmed positive drug and/or alcohol test, or a refusal to test will result in disqualification and/or termination from employment with Wyoming Machinery Company [see note (2) below].

I certify that the information I have provided in this application is true and correct, and I release all parties from liability for any claims or damage that may result from providing or receiving information in support of this application. I understand that misrepresentation or omission of facts called for in this application is cause for immediate disqualification from possible employment and dismissal from any actual employment with WMC without recourse.

I understand that I am required to comply fully with all personal identification and employment eligibility requirements of the Immigration Reform and Control Act and that failure to do so will result in termination. If hired, I will be required to furnish proof of eligibility to work in the United States. In addition, I understand that Wyoming Machinery Company uses the “E-Verify” program to verify employment eligibility of newly hired employees.

If employed, I agree to abide by all present and future policies and procedures of Wyoming Machinery Company and applicable state and federal regulations. If employed, I understand that my employment is for an indefinite period of time, that I have not been promised continued employment by Wyoming Machinery Company for any definite, specified period of time, and that my employment may be terminated at any time for any reason without prior notice or discipline. In the event that the Company advances me money or other items of value, or I otherwise become indebted financially to the Company, I agree to repay the Company. Any wages due to me upon termination may be offset by payroll deduction against such monies due to the Company.

I understand that completion of this employment application form does not constitute any type of employment agreement or contract.
